Damaged pavement dangerous for wheelchair

Sent to Lincolnshire County Council 3 minutes later. Council ref: 350779.
The pathway leading up to the train tracks is full of chipped and missing pavement which is damaging the wheelchair that belongs to the lady I care for. There is no way to cross the road safely. She also shouts in pain as we go over it due to the bumping around. She is wheelchair bound and relies on this path so we can take her to town and the river.
