Collapsing man hole cover

Sent to Lincolnshire County Council less than a minute later. Council ref: 378157.
The rectangular steel man hole cover was replaced recently after becoming deformed. Unfortunately it is deformed again with a high probability of collapse as it is at the point that all vehicles turn into Carre St. I observed a 6-wheeler transit the cover just recently.

This issue is getting worse since the nights have drawn in - made worse with the deteriorating weather the position and road conditions could well lead to a very dangerous situation with serious injury - a busy place for vehicles and pedestrians using the crossing including primary age children and the elderly, urgent action is required thank you.

This collapsing man hole cover is getting worse. To add to the risk, a large subsidence hole has developed adjacent to it. This hole and the n the ridge of the collapsed man hole causes a severe jolt to the front on-side wheel of passing cars whilst the wheel is turned to the right. Motor cycles and bicycles would be at more risk of losing control but damage to front suspension and track rods of cars is highly likely. The council response of no further action is not acceptable..
