Uneven, slippery pavement on slope outside church gate

Sent to Lincolnshire County Council less than a minute later. Council ref: 378330.

The section of pavement outside the church gate to the north-east is cracked and uneven, and is on the site of the former village pump, meaning it is often wet and therefore slippery. As a result of slipping on it, a member of Ingoldsby's Parochial Church Council slipped and fell on Thursday 8th October (in daylight), injuring herself and requiring hospital admission. We'd be grateful if it could be mended as a matter of urgency, as most of our small congregation are elderly. The church is also used by other village residents. Many thanks!
