Poor Repair Not Resolved issue

We have been having an issue with potholes in the middle of the street, which also seem to be allowing a stream of water to drain down the centre of the street. You will see that this has been an issue due to previous reports and your attempt of patching this with some tarmac hasn't cured the issue. This problem does need to be addressed immediately as if this is left until the end of the year, when the temperature drops and we experience icy weathers the issues becomes a major concern for peoples health and potential damage to vehicles as the stream of water turns to a sheet of ice. Last winter we saw many incidents where people slipped on this and hurt themselves, even a Sainsburys delivery driver going about there work. We also experience a lot of people trying to force their cars up the street, slipping all over on the ice as this is a very steep hill.
I will be adding images to show the issue in more detail, but please may a more significant repair be taken out as this is issue can be very dangerous at certain times of the year.
