Trip hazzard on pavement

Sent to Lincolnshire County Council less than a minute later. Council ref: 407073.

I was walking along usher avenue this morning as i was nearly at the top of the road my foot caught a paving slab that was sticking up and i fell injuring both hands, one of my knees and now have a very sore and painful right shoulder. we have lived in this area for nearly 3 years and its been a hazard in all tht time and nothing seems to improve
