Trip hazard/uneven surface problem

Sent to Lincolnshire County Council less than a minute later. Council ref: 4169185.

There is a concrete cover with a metal rim in the footway where the concrete has come away - approx 2ins wide & 3 ins long & 1 in deep. Wife had a fall here a week last Tuesday - she got the heel of her boot stuck in the gap. Please investigate.
