Large rise in pavement about 5cm above level

Sent to Lincolnshire County Council less than a minute later. Council ref: 416096.

Pavement has risen about 5cm above the level on either side of it. I tripped on it and my wife tripped on it in the dark two nights ago. No injuries but plenty of potential for injury. Outside No 9 Towngate West. Not sure how long it's been there but I recall it being not right before Christmas.
