Numerous areas of damage/uneven surfaces on a stretch of footpath.

This has been deteriorating further over the last year. It is a widely used stretch of pavement on harris boulevard, covering a LONG STRETCH BETWEEN NUMBER 22 and the house on the corner of the junction with Seacroft Road. My elderly mother caught the toe of her shoe in one of the defects IN March, and if I'd not been with her to prevent her falling forwards, she would have fallen and most likely badly injured herself. I HAVE REPORTED THIS PROBLEM NUMEROUS TIMES BEFORE, OVER THE LAST 5 YEARS!
