There is a manhole and patch. The patch is about 1.2m square. Underground to the right, south side, of the manhole cover appears to be collapsing. The manhole itself is not level. It is right where truck wheels will pass.

You misidentified this as report 482048. It was recorded as a new report 481474. I hope the photograph helps identify the right hole.
