Pothole on road/cycleway problem

Customer wishes to report pothole and make a claim. 'When traveling eastbound on the A52 heading home, road ahead clear, I accelerated to pass a slower moving vehicle, at approx 55mph. Whilst on the other side of the road, in process of passing this vehicle, I drove into a series of continuous pot holes causing extensive vibration, banging noise and deviation of the steering wheel to the right causing me to veer moreso to the right. This caused me to ride the verge for seconds before I could pull back in onto the road. I recall a scary, noisy, being jolted up and down and airborne debris.
Fortunately I was able to regain control of the car, remaining on the "wrong side of the road" and breaked hard and luckily no other vehicle was coming on the A52 westbound - I then pulled into the small road to compile myself noticing the window crack.'
