Pothole on road/cycleway problem

Customer wishes to report pothole and make a claim. 'I was on North Street waiting for the traffic lights to turn green, and I was in the lane to turn right. I was the first car. When the light turned green, I set off at around 5mph, and I was en-route and turning right onto the A607 in the far lane heading to New Street. I felt a bump on my driver's rear end. It was not safe to get out of my vehicle after the damage had occurred as there were many cars around and this is a busy road, when I was en-route to safe location where I could assess the damage to my car, I heard an awful, loud, crack. I got out the car and I could see the car was noticeably lower at the rear driver's side. I drove slowly to the garage to get my car fixed.'
