Pothole on pavement/verge problem

Customer wishes to report uneven footway surface and make a claim. 'Went unbalanced on the uneven surface of the path. Landed head first onto the ground with my nose hitting the ground first followed by my full body including my knees. The pathway is uneven do to the ground cracking which in turn makes the surface uneven to walk on.'
